FDA & HEALTH AUTHORITY INSPECTION READINESS SIMULATION
We don’t just teach inspection readiness. We simulate it, so confidence holds when the pressure is real.
Inspection readiness training grounded in neuroscience and performance science. Through realistic FDA and Health Authority simulations built on Stress Exposure Training (SET) and Stress Inoculation Training (SIT), your people learn to stay calm, think clearly, and build trust and rapport with inspectors, for a more professional inspection and a better outcome.

INSPECTION OUTCOMES
Calmer rooms. Clearer answers. Stronger outcomes.
Inspection-ready participants
People who walk in prepared and composed, ready to show the quality work they have done.
Team readiness
Better team cohesion and trust, while staying calmer under pressure.
Performance
Decisions you can defend, communicated clearly and confidently.
WHY PQS EXISTS
Pressure exposes gaps. Knowledge alone doesn’t close them.
Over 30 years in engineering and pharma/biotech quality, we watched the same pattern play out again and again: talented, hardworking teams walking into inspections already braced for the worst. Treating it like a trip to the dentist. Dreading it instead of owning it.
Under the stress of an audit or a launch, pressure pulls people off their peak. SMEs overshare or shut down. Tense moments with inspectors escalate. Simple questions turn into findings.
That gap in preparing the individual’s actual performance, not just systems or audit knowledge, is exactly what PQS was designed to close.
“
No one should fear an inspection. It’s a chance to show off all the quality work you’ve done to keep patients safe and deliver great treatments. That’s the mindset shift we help teams make."
Paul Hefner
Founder & Lead Facilitator, PQS
WHAT ONLY PQS DELIVERS
Not more slide decks. Not stress-testing for stress’s sake.
Most inspection training falls into two camps: classroom content about what inspectors want, or mock audits designed to expose organizational weaknesses. PQS trains the people who sit across from the inspector, so those conversations go better: calmer, clearer, and more productive.

A SAFE PLACE TO PRACTICE
Real pressure. No real-world consequences.
People learn differently when they’re relaxed, and perform differently when the stakes are high. PQS gives participants a safe space to feel inspection pressure, experiment, make mistakes, and recover, so the first time they feel that pressure isn’t in front of a real inspector.
1
Built to build people up
The goal is never to catch anyone out. Simulations strengthen confidence rather than expose weaknesses.
4
Every seat, every role
Roles rotate (SME, scribe, strategy lead, host/escort) so each person sees the inspection from every side of the table.
2
Progressive exposure
Following Stress Inoculation Training principles, pressure increases step by step as skills and confidence grow.
5
Small, focused cohorts
Twelve participants per instructor means real attention, real feedback, and time to practice.
3
Coaching after every scenario
Facilitated debriefs turn each simulation into specific, practical takeaways.

PROGRAM OVERVIEW
“We don’t just teach inspection readiness. We simulate it.”
PQS is the only inspection readiness training built on both regulatory expertise and performance science. Participants don’t sit through slides. They navigate live inspector interactions, perform real audit request tasks, find planted document errors, and work through high-pressure scenarios that mirror real FDA and Health Authority audits.
With our SET/SIT-based methodology, participants gain the mindset, stress management tools, and mental resilience to move from uncertain and anxious to prepared and confident. Inspections aren’t just a test of knowledge. They’re a test of how your team holds up when product approvals and patient outcomes depend on every answer.
The result: teams that don’t just know what to do, but are mentally ready to perform and work well together when it counts most.

RESULTS HIGHLIGHTS
Data-forward proof for high-stakes inspection readiness
PQS combines stress exposure training with realistic inspection simulations to deliver confident, credible performance when it matters most.
RESEARCH INSIGHT
2x
stronger effect when methods are combined
A 2024 meta-analysis of 35 studies (5,600+ participants) found stress-reappraisal and mindset training reliably improves performance under pressure. Effects were largest when the approaches were combined and the task was public and high-stakes, like an inspection interview. PQS is built on that combined approach.

RESEARCH INSIGHT
3 in 4
A meta-analysis of 37 studies (1,800+ participants) found stress inoculation training significantly reduced anxiety and improved performance under stress. The typical trained participant outperformed about three out of four untrained peers. PQS simulations are built on these stress inoculation principles.

THE BUSINESS CASE
What is one avoidable observation worth?
Many 483 observations and stressful inspection moments don't start with a quality problem. They start with communication. An unclear answer, an overshared detail, or a document walkthrough that goes sideways can lead to unnecessary questions, additional information requests, expanded scrutiny, inspection delays, and increased pressure on approval and launch timelines.
Protect timelines
Composed, credible SMEs help keep inspections on track and approvals and launches moving.
Reduce remediation effort
Fewer misunderstandings mean fewer concerns to investigate, respond to, and close out.
Build lasting capability
Skills and mindset last beyond inspections and audits, carrying into launches, leadership meetings, and any high-pressure moment at work or in life.
MEASURED, NOT ASSUMED
See the change your team made.
We baseline each participant’s confidence, stress, and dread about inspections before training, then measure again afterward. You get evidence that the investment worked, not just a certificate of attendance.

David Grote
Senior Quality Advisor · Fractional Quality Leader · Former Head of Quality, Immunocore
David brings more than 30 years of biopharmaceutical operations, manufacturing, and quality leadership to the companies he advises. Before consulting, he held validation, quality, and operations leadership roles at Amgen, Teva, and Immunocore. He has led quality systems for clinical and commercial manufacture of biologics and sterile products, and has guided programs to approval with FDA, EMA, and other health authorities. He has prepared sites for FDA and global health authority inspections, managed inspections as they happen, and led the response and remediation that follow.
His technical specialty is investigating and resolving microbial, particulate, and cross-contamination events in sterile and non-sterile manufacturing, including contamination control strategy under EU GMP Annex 1 and FDA aseptic processing guidance. He also provides fractional quality leadership and supports quality due diligence for M&A and private equity transactions. That experience shapes how he teaches inspection readiness at PQS: grounded in what inspectors actually look for and in how a site's people, documents, and systems hold up under questioning.

FAQ
Answers for inspection-ready teams
Need a specific answer? Email phefner@positivequalitysolutions.com
Who should attend?
QA, QC, Manufacturing, Validation, Laboratory, Engineering, Regulatory, CMC leadership, inspection leads, and anyone who wants to be mentally prepared for internal audits or FDA and global Health Authority interactions.
What does the simulation include?
Live inspector interactions, role rotations, planted document errors, mock documents modeled on recent FDA 483s (including records you must defend without having written them), real-time coaching, and a safe but high-pressure environment. It runs just like an actual inspection, in a space where you can experiment and learn.
Is this for beginners or experienced teams?
Both. We calibrate scenarios for new contributors and seasoned SMEs so every role is challenged and supported at the right level.
Will this feel like a stress test?
No. Pressure is introduced safely and step by step, and every scenario is followed by coaching and a debrief. Anyone can pause a scenario at any time, something you can't do in a real inspection. Our number one goal is to build your confidence and give you better strategies, not to expose weaknesses.
How is SET/SIT different from typical training?
Knowing a procedure in a conference room and executing it during a live FDA audit are not the same skill. Stress Exposure and Stress Inoculation Training deliberately and progressively introduce realistic pressure, so participants build the decision-making habits and emotional regulation that hold up when it matters.
How do we know it worked?
We baseline participants’ confidence, stress, and anxiety about inspections before the course and measure again afterward, so you can see the change.
Can teams register together?
Yes, and we encourage it. Contact us for group pricing. Teams that attend together practice roles, handoffs, and room dynamics as a unit.
What should I bring?
Laptops are mandatory: you’ll use it to prepare, review, and defend simulated documents.
Is parking available?
Yes, free and secure onsite parking is available,
Are hotel rooms available onsite?
Yes, the training is held at the hotel, with rooms available onsite for anyone who needs to stay overnight.
How does this reduce the risk of a 483?
Teams practice high-pressure inspection behaviors, sharpen their responses, and learn to avoid the communication missteps that can turn a manageable question into a concern, and a concern into an observation.
What is the cohort size?
Cohorts are capped at 12 per instructor, to maximize feedback, realism, and active participation.
Is lunch included?
Yes. Lunch and light refreshments are included for in-person sessions, and dietary needs can be accommodated with advance notice.
What if I have to defend work I didn’t do?
That’s one of the hardest inspection situations, and we practice it directly. When it’s your own work, you remember the history, the conversations, and why decisions were made. When it’s a colleague’s work, or from before you joined, you’re limited to what’s in the documents. Participants work through mock records modeled on recent FDA 483 observations, piece together the history and rationale as best they can, and build a strategy they can defend in front of an inspector.
How do I get my manager’s approval?
Download the one-page manager brief for the February 2-3 class in the Course A Agenda section above. It covers the agenda, ROI, logistics, and pricing, so your manager has what they need to approve your attendance.
